Screening Tests in Stroke Patients before Cognitive Perceptual Assessment for Driving

Abstract
Objective: This study was to evaluate Clock Drawing Test (CDT) and Korean Mini Mental State Examination (K-MMSE) in predicting the Cognitive Perceptual Assessment for Driving (CPAD) in stroke patients and elderly stroke patients.

Methods: 150 stroke patients were evaluated with CDT, K-MMSE and CPAD. We defined CPAD Pass group as a CPAD score of ¡Ã 53 point and Fail group as a score of <53 point. After adjusting for potential confounding factors in the multivariate analysis, we calculated the odds ratios (ORs) and confidence intervals (CIs) of CDT and K-MMSE in the prediction of CPAD group.

Results: Among 150 patients, 82 (55%) were classified as CPAD pass group. CPAD Pass group was significantly associated with age, total CDT score, CDT subgroups (spacing, time), total K-MMSE score and K-MMSE subgroups (place orientation, attention, recall). But CPAD Pass group in patients aged ¡Ã55 years was significantly associated with total CDT score, CDT subgroups (time), total K-MMSE score and K-MMSE subgroups (place orientation, attention) and for patients aged ¡Ã65 years, no statistically significant association was found. In the multivariate analysis, age (OR, 0.924; 95% CI, 0.883-0.967, p=0.001), time of CDT (OR, 8.538; 95% CI, 2.462-29.611, p=0.001) and K-MMSE total score (OR, 1.510; 95% CI, 1.234-1.849, p<0.001) predicted CPAD pass group after adjusting for potential confounding factors in 150 patients. K-MMSE total score (OR, 2.500; 95% CI, 1.358-4.602, p=0.003) only predicted CPAD group in patients aged ¡Ã55 years.

Conclusion: The CDT and K-MMSE are time-effective tool in predicting the CPAD for identifying stroke drivers.
